Box 650, Madison, NE 68748 and advise us of your privacy choices by providing your name, address, telephone number and account numbers and whether you request to limit the personal information about you that we share with nonaffiliated third parties (Choice 1) or Madison County Bank affiliates (Choice 2), or both (Choice 1 and 2). Be sure to include account number(s) in your privacy choices, since your privacy will be applied only to the accounts which you identified.
